Development of experimental allergic rhinitis model using Japanese cedar pollen.

および鼻閉を主症状とする典型的な I型アレルギー疾 患である.くしゃみおよび鼻汁分泌の亢進は,抗ヒス タミン薬により抑制される.一方,患者にとって最も 苦痛を強いられる鼻閉にはグルココルチコイドが繁用 されるが,周知のごとくグルココルチコイドは優れた 効果を発揮するが副作用も強く,これに代わる鼻閉の 治療薬が望まれている.実験動物を用いたヒトの病態 を反映した良好なモデルは,新規治療薬の開発に極め て重要な位置にあることは論をまたない.そこで,我々 はモルモットを用いてスギ花粉抽出エキスで経鼻的に 感作し,以後スギ花粉の反復吸入を行うことにより, ヒトの病態に類似した症状を呈するアレルギー性鼻炎 モデルの開発を企図した.本法により,抗原惹起後に 明らかなくしゃみおよび二相性の鼻閉が認められ,く しゃみの発現は周知のごとく抗ヒスタミン薬で強力に 抑制された.一方,遅発性の鼻閉は,システイニルロ イコトリエン(CysLT)の受容体拮抗薬で強く抑制さ れ,さらには遅発性に明らかな CysLTsの産生が認め られた.また,一酸化窒素(NO)合成酵素阻害薬が 遅発相を抑制したことから,遅発性に産生された CysLTsが NOを介する血管拡張により遅発性の鼻閉 を誘起しているものと推察された.さらに,本モデル の特徴は,慢性および重症化した患者において認めら れる鼻過敏性が認められることである.本鼻過敏性の 発症は,ブラジキニン B1 および B2 受容体に対する 拮抗薬で抑制され,さらにはこれらの受容体のアゴニ ストである des -Arg -カリジンおよびブラジキニンの 点鼻は感作―惹起動物においていずれも鼻過敏性を誘 起することから,本鼻過敏性の発症にはキニン類が大 きな役割を演ずる可能性が示唆された.このように, 本モデルはヒトのアレルギー性鼻炎の病態を良好に反 映しており,薬効評価さらには病態のメカニズム解析 において有用であると考えられる.
